Premium, deductible, copay — what's the difference
Your premium is what you pay monthly to have insurance, whether or not you use it. Your deductible is what you pay out-of-pocket before insurance starts covering services. A copay is a fixed amount you pay at each visit, regardless of the deductible. Your out-of-pocket maximum is the most you'll pay in a calendar year — after that, your plan covers 100%. In-network vs. out-of-network — the real cost
In-network providers have a negotiated rate with your insurer — meaning you pay a fraction of the actual cost. How to read your Explanation of Benefits
Your EOB (Explanation of Benefits) is what your insurance sends after a claim is processed — it's not a bill. It shows what was billed, what your insurer paid, and what you owe. The key column is "patient responsibility." If that number doesn't match your bill from AymanCare or any other Dallas-area provider, contact both your insurer and the provider before paying. Billing errors are common and correctable.
